Pubalgia is a painful syndrome, sometimes invalidating, involving agonist and non-agonist sportsman. It represents 2-5% of sport-related diseases in agonist sportsman and its cause is not always easily diagnosed. Osteitis pubis represents one of its more frequent causes involving sportsman in 0.5-0.7% of cases. Between September 2003 to August 2004, 40 athletes, 37 men and 3 women (age range 18-37 years; mean age: 24 years), with clinical evidence of pubalgia were submitted to pelvic MR. In all cases symptoms were unilateral (33%) or bilateral (67%), irradiating to adductor region and to rectus abdominis muscle, presenting ingravescent evolution. MR shows primitive pubic osteitis in 21 cases (5 cases early stage and 16 cases in late stage). Changes of pubic bones and myo-tendinous junctions were assessed in 16/16 cases in late stage. Muscle involvement was observed in 19/21 cases and cortical edge changes were assessed in 12/16 cases. Only in one case we observed extant joint effusion and a millimetric calcification. RM, due to its intrinsic characteristics of high contrast resolution and multiplanarity, is the most suitable imaging technique allowing osseous, capsulo-ligamentous, tendinous and muscular changes evaluation and its balance of extension in early and late stages, permits a correct therapeutical planning and follow-up

La pubalgia è una sindrome dolorosa, a volte a carattere invalidante, che colpisce sia gli sportivi agonisti che quelli amatoriali, rappresenta il 2-5% delle patologie correlate all’attività sportiva agonistica e non sempre le sue cause sono facilmente diagnosticabili. L’osteite pubica rappresenta una delle principali cause di dolore inguinale e colpisce gli sportivi nel 0,5-0,7% dei casi. Dal settembre 2003 all’agosto 2004, 40 atleti, 37 uomini e 3 donne (età compresa tra 18 e 37 anni; età media: 24 anni), con diagnosi clinica di pubalgia sono stati sottoposti a RM del bacino. In tutti i casi la sintomatologia era caratterizzata da dolore mono (33%) o bilaterale (67%) con irradiazione alla regione adduttoria e al muscolo retto dell’addome e andamento ingravescente. La RM ha evidenziata in 21 casi la presenza di osteite pubica primitiva (5 casi in stadio precoce e 16 casi in stadio avanzato). La presenza di alterazioni delle ossa pubiche e delle giunzioni miotendinee è stata evidenziata in 16/16 pazienti classificati in stadio avanzato. L’interessamento muscolare è stato osservato in 19/21 casi. Le alterazioni del profilo corticale sono state osservate in 12/16 casi. Soltanto in un caso si associava una cospicua quota di versamento articolare e una millimetrica calcificazione. La RM, in relazione alle intrinseche caratteristiche di elevata risoluzione di contrasto e di multiplanarietà, risulta la metodica più idonea consentendo lo studio delle alterazioni delle strutture ossee, capsulo-legamentose, tendinee e muscolari e della loro estensione sia nelle forme precoci che in quelle tardive, in funzione sia di una corretta pianificazione terapeutica che di un eventuale monitoraggio del trattamento.
