This paper investigates the waste production in a large hospital facility specialized in organ transplants: Ismett (Istituto Mediterraneo per i Trapianti e Terapie ad Alta Specializzazione), which is located in Palermo (Italy). Relations were determined between waste quantities and composition on one hand; the complexity of the sanitary structure and the characteristics of treatments provided on the other. The relevant technical Literature contains – almost exclusively – data from surveys made in General Hospitals. These data can be overall – i.e. referring to all operational units present in the structure – or for individual Departments. This study has the peculiarity of processing data on the production of waste in a Hospital dedicated to one type of health care service, namely the transplants and related therapies. With the effective contribution of the Institute, data covering three years (2010-12) were examined and processed. After calculation of waste production indexes it was possible to compare the Ismett with other healthcare facilities. During this investigation data were drawn concerning: active departments, production of infectious waste per year, number of beds on duty, number of admissions and number of stay’s days. Starting from this data, useful parameters were calculated for studying healthcare waste generation rate. In detail these are: bed occupancy rate, average length of stay and number of occupied beds. To obtain general information it was calculated the waste production indicators internationally used, which relate the healthcare treatments allocated with the waste production and allow comparisons among different sanitary facilities. Drawing from a study of the year 2006 (Nicosia et al., 2006), it was possible to compare the value of the indexes calculated for Ismett with those of four hospitals in Palermo. It was noted a step of one order of magnitude between infectious waste production at Ismett, and the corresponding production in other healthcare facilities; the differences found probably descend from the procedures and the in-patient protection standards put in practice at Ismett, and on the complexity of the operations carried out. The complexity of treatments provided to in-patients is also involved in assessment of healthcare facilities efficiency. To this some indexes of sectors were calculated, which confirm the specificity of Ismett as healthcare facility.

Questo studio analizza la produzione di rifiuti a rischio infettivo in una grande struttura ospedaliera specializzata per le attività di trapianto: Ismett (Istituto Mediterraneo per i Trapianti e Terapie ad Alta Specializzazione), che si trova a Palermo. Sono state determinate le relazioni fra la quantità e la composizione dei rifiuti, da una parte; la complessità della struttura e la natura delle cure erogate, dall’altra. In letteratura si trovano regolarmente i risultati di ricerche sulla produzione di rifiuti a rischio infettivo nei più diversi Paesi del mondo; i dati pubblicati provengono quasi esclusivamente da Ospedali Generali, anche complessi e di Alta Specializzazione. Tali dati possono essere complessivi – cioè riferiti a tutte le Unità Operative di cui è dotata la struttura – o relativi ai singoli Reparti. Lo studio qui presentato ha la peculiarità di elaborare dati sulla produzione di rifiuti in una struttura ospedaliera interamente dedicata ad un tipo di prestazioni: trapianti e terapie correlate. Con il fattivo contributo dell’Istituto sono stati elaborati i dati di tre anni di esercizio (2010-12), e usando gli indicatori di produzione è stato possibile confrontare questa struttura con altre. Durante l’indagine sono stati rilevati dati riguardanti: reparti in funzione, produzione di rifiuti infettivi, numero di posti letto attivi, numero di ricoveri e di giornate di degenza. A partire da questi è stato possibile calcolare i parametri di riferimento per la produzione specifica di rifiuti sanitari, in particolare: il tasso di occupazione dei posti letto, la durata media della degenza e il numero di posti letto occupati. Per ottenere indicazioni di valore più generale, si sono calcolati alcuni indicatori della produzione dei rifiuti − internazionalmente utilizzati − che legano la produzione di rifiuti alle prestazioni di assistenza e consentono di effettuare confronti tra diverse strutture sanitarie. A partire da uno studio del 2006 (Nicosia et al., 2006) è stato possibile comparare i valori degli indici calcolati per l’Ismett e quelli di quattro strutture ospedaliere di Palermo. È stato rilevato un salto di un ordine di grandezza fra la produzione di rifiuti infettivi per paziente all’Ismett e quella negli altri Ospedali; le differenze riscontrate dipendono verosimilmente dalle procedure e dagli standard di protezione dei pazienti messi in atto all’Ismett, e dalla complessità delle operazioni effettuate. La complessità delle prestazioni erogate ai pazienti interviene anche nella valutazione dell’efficienza di una struttura sanitaria. A tale scopo sono stati calcolati alcuni indici di settore che hanno confermato la specificità dell’Ismett come struttura sanitaria.
